AB In June 1993 a serum sample from a high-risk patient was referred to the
Victorian HIV Reference Laboratory at Fairfield Hospital for routine
anti-HIV testing. The serum was repeatedly reactive (S/C0 = 3.7) by the
Abbott HIV-1/HIV-2 EIA. Confirmatory testing by HIV-1 Western blot
showed an indeterminate profile with moderate-weak reactivity to P24,
P34 and P55. The serum was strongly reactive to HIV-2 in the Genetic
Systems GENIE synthetic envelope peptide assay and was non-reactive to
HIV-1. The specimen was confirmed positive by HIV-2 Western blot at the
National Reference Laboratory, showing reactivity to all HIV-2 bands.
The serum was non-reactive by the Wellcome HIV-1 EIA and the AGEN HIV-1
SimpliRED assay. Retrospective testing of a July 1991 serum from the
patient was non-reactive in the HIV-1/HIV-2 ELA. Although HIV-2
infection is extremely rare in Australia, this case illustrates that the
potential for its spread obviously exists. It is reassuring that
Australian diagnostic and blood bank laboratories are now using
screening assays capable of detecting HIV-1 and HIV-2 infection.
